A 60 male presented with a strongly positive TMT. Coronary angiography revealed a *distal left main (LM) plaque and critical 90% stenosis at the ostium of the left anterior descending artery* (LAD).
Given the complex anatomy and high-risk location, the patient underwent a precision PCI using IVUS guidance—from the left main to the LAD successfully
